Key responsibilities of the Nurse Practitioner include:
  • Providing direct care to aging residents in long-term care facilities
  • Collaborating with primary care physicians, facility staff and patients' families
  • Participating in care conferences with other providers and patient family members
  • Working with the facilities utilization review process
  • Identifying patients' behavioral health issues
  • Educating nursing home staff, physicians and administration
  • Coordination of care for patients

Qualifications
Requirements:
  • Nationally certified Nurse Practitioner status
  • Certification in Adult, Family or Gerontology
  • Licensure by the State of WI
  • Proficiency in Computer Skills
  • Prescribing rights 
 
In addition, assets that would be beneficial include:
  • Clinical experience
  • Long-term care experience
  • Knowledge of mental health issues
  • A background in managed care
  • Familiarity with Medicare and ICD-9 coding
