Certified Home Health Aide - Concord, CA
 
Primary Responsibilities: 
  • Providing the patient assistance with ambulating, transfers, positioning, exercising, feeding and other personal care tasks
  • Taking vital signs, changing simple dressings, assisting patients in taking their medications and performs other duties as assigned by the RN
  • Performing light housekeeping duties such as preparing meals and changing sheets
  • Reporting changes in patient's condition to RN Case Manager
  • Submitting CNA visit records in a timely manner
  • Regularly attending departmental meetings

Qualifications

Qualifications: 
  • Active Home Health Aide certificate is required.
  • 1+  years of experience working as a Certified Home Health Aide is required.
  • Experience in providing activities of daily living for terminally ill patients.
  • 1 + years experience providing care in patient's homes is required.
  • Experience with Word, Excel, and the Internet are highly preferred.
  • Reliable transportation, insurance and clean driving record is required.
  • Great communication skills and a passion for people.
