Job Description
 

Part-Time, 40-48 hours per pay period. Shift: 8:00am-5:00pm 3 days per week including night, weekend & holiday call.

The Registered Nurse in the Transplant Coordinator role will be responsible for coordinating a focused and effective effort between the transplant center and its major patient referral sources for renal transplantation. The position will assume an account manager role in assuring that potential transplant recipients and donors will be afforded timely; efficient transplant education, work up, updates, and activation for transplantation when indicated. The incumbent will establish an open and consistent dialogue with the respective dialysis facility staff and will be the key contact with the Transplant Program. Aspects will include coordination of referrals, responding to roadblocks within the transplant workup process, and providing patient, family, and staff educational programming. The incumbent will work under the direction and in collaboration with the Administrative Director of Transplant to assure policies, procedures, and all regulatory requirements are met to assure the transplant program stays in compliance with various governing agencies. This position will be responsible for a designated caseload of patients during the peri-transplant process. Assignments may include carrying out clinical and data collection activities within the transplant program, direct patient and family interface and interactions with the dialysis staff who may have supplemental learning needs related to transplantation. May provide a supportive role in advising the regional dialysis programs with concerns related to the transplant work-up.

Job Knowledge and Skills:  

Following formal orientation, competency in the assigned clinical area is expected. Must demonstrate advanced clinical knowledge in nephrology and/or solid organ transplant, and have the ability and/or expertise to determine appropriate methods of meeting the varying levels for patient/family learning needs. Must have a vision for the integral role this position contributes to the entire programmatic approach to transplantation. Must possess a high level of critical thinking, reasoning skills, and positive, assertive communication skills.

The RN functions within the scope and standards of nursing practice as outlined in the South Dakota Nurse Practice Act and Administrative Rules. The Sanford Nursing Practice recognizes the Scope and Standards of Practice (2004) and the Code of Ethics for Nurses with Interpretive Statements (2001) as published by the American Nurses Association as the foundation for nursing care delivery.

Required Skills

Graduation from a NLN or AACN accredited nursing program is strongly preferred. Graduate from an approved nursing program is acceptable. Bachelor's Degree in nursing preferred.

Must fulfill yearly continuing education requirements according to policy. Must complete orientation for various procedures and other in-services as required by the unit Education Council and hospital policy. Currently licensed with applicable State Nursing Board and/or possess multistate licensure privileges as required by position.
