Description

Under general supervision, provides pharmaceutical care by performing various patient focused clinical, educational, informative, & distributive functions in inpatient and/or home infusion as interdisciplinary team member in support of patients, providers, & ancillary staff. Ensures appropriate, cost-effective drug utilization while maintaining & enhancing patient care.

Essential Functions:
Prepares & dispenses medications using appropriate techniques & applicable policies & procedures
Researches & organizes patient-specific information; documents patient medication histories; recommends or reviews laboratory tests to assess patient response to drug therapy; identifies medication therapy problems as appropriate
Designs appropriate pharmacotherapeutic regimens; integrates patient-specific disease & drug information, current practice standards, ethical issues, & quality-of-life issues, & pharmacoeconomic principles
Recommends pharmacotherapeutic regimens & corresponding monitoring plans to prescribers in way that is systematic, logical, & secures consensus from prescriber & patient; utilizes patient interview/assessment skills, current literature-supported evidence & practices, drug initiative information, established protocols
Monitors data & modifies pharmacotherapeutic plans as necessary; documents pharmaceutical care activities where appropriate
May perform limited physical examinations, such as blood pressure & glucose measurement, for the purpose of providing medication management services
Documents pharmaceutical care activities in patients' medical records or clinical information system
Establishes relationships w/ physicians & facility leaders to successfully partner on drug therapy issues; balances cost & quality issues
Participates in establishing disease state management protocols in coordination w/ HPMG physicians, Clinic Nursing, & Pharmacy & Therapeutics committee
Provides patient-specific verbal or written consultations, including pharmacotherapy & pharmacokinetic, to healthcare practitioners
Provides pharmaceutical care services for defined high risk patient populations; ensures continuity of pharmaceutical care & optimal utilization of resources to & from acute & ambulatory patient-care settings; collaborates w/ inpatient/outpatient pharmacist or established regional disease management programs for follow-up w/ high-risk discharges
Designs effective medication-use education for patients & caregivers; provides one-on-one counseling to patients & caregivers, including information on drug therapy, adverse effects, compliance, appropriate use, & handling
Analyzes requestor's drug information need; employs systematic, efficient, & thorough procedure for retrieving drug information or enlists available resources; formulates response; follows up consistently on drug information recommendations & assesses effectiveness
Participates in monitoring & improving relevant outcome measures and/or quality improvement indicators
Performs other duties & accepts responsibility, as assigned

Qualifications

Basic Qualifications:
Graduate from an accredited pharmacy college recognized by the American Council of Pharmaceutical Education (ACPE)
Completion of an emergency contraception training course approved by the American Council of Pharmaceutical Education (ACPE), curriculum-based programs from an ACPE-accredited college of pharmacy, applicable state or local health department programs, or programs recognized by the board of pharmacy
Valid Hawaii Pharmacist license (must meet education requirement(s) for Hawaii State licensure)
Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification
Demonstrated ability to safely handle and/or prepare various pharmaceutical compounds (e.g., chemotherapeutic agents, biologicals)
Demonstrated knowledge of and skill in adaptability, change management, customer service, decision making, interpersonal relations, oral communication, problem solving, quality management, teamwork, and written communication
Demonstrated knowledge of and skill in word processing, spreadsheet, and database PC applications
Talking to coworkers, customers, outside vendors, and on the telephone
Reading, writing, speaking, and understanding English
Training/giving and receiving instructions
Mathematical ability, attention to detail (e.g., organization, prioritization, proofing), concentration, and alertness

Additional for Pharmacist Clinical II - Inpatient:
Two years of full-time practical (post graduate) experience or completion of one year general residency
Demonstrated knowledge of related federal, state and local regulations and accreditation requirements including JCAHO standards

Preferred Qualifications:
Doctor of Pharmacy Degree from a school recognized by the American Council of Pharmaceutical Education (ACPE)
Completion of a general residency program
Board Certified Pharmacotherapy Specialist (BCPS) by the Board of Pharmaceutical Specialties (BPS)
