- Responsible for driving the FMS culture through customer service standards.

- Accountable for outstanding customer service to all external and internal customers, including patients, staff, physicians, clinical managers, area managers, business unit and corporate personnel and payors, including disease management entities.

- Develops and maintains effective relationships through effective and timely communication with all customers.

- Takes initiative and action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner.

- Strong patient advocate for treatment choice.

- Knowledgeable about all dialysis modalities.

- No pre-conceptions about which patients are able or candidates about any given modality

- Able to present material clearly and without bias.

- Able to discuss openly no therapy if patient(s) desire.

- Motivated to record data and fully engaged to follow up as needed to ensure the programs success.

- Good communication skills; ability to present to a potentially large crowd.

- Ability to engage the various emotional issues presented by patients in a calm, empathetic, and supportive manner that diffuses tension.

- Able to devote approximately 6-8 hours for Treatment Options Education activities per pay period; actual time unknown at this time.

- Able to maintain some flexibility in schedule to meet individual patient needs when necessary.

Work with Area Manager to determine the following:
* Location/site of primary Treatment Options Program
* Coordination of extended service to geographically

remote facilities within your area; ie; work with remote facility CM, SW,

Dietitian, etc to provide Treatment Options Education
* Schedule for Treatment Options class (day& time to be pre-determined)
* Market the program to the local nephrologists offices & hospital discharge

Planners with the Area Manager

. Conduct primary Treatment Options Education class; minimum of 1/month.

. Complete data collection template for attendees.

. Follow up phone call to attendees within one to two weeks following class to

collect option(s) of choice and document. Forward data collection form to referring

physician.

. Follow up with phone call to patient on monthly basis until patient begins ESRD

or other.

. Provide Area Manager with pertinent information regarding successes and issues

with program. May consider drafting a report for CQI use.

- Responsible for ensuring effective adult learning environments for all educational programs.

- Supports Business Unit, Regional, Area and facility-specific business plans.

- Maintains knowledge of FMNCA products, services, strategies, Ultra Care, and Options Modalities in order to support and train patients appropriately.

- Any other special projects as assigned by the Regional Director of Home Therapies and the Area Managers, RVPs, etc.

Requirements
 

- At least 1 year experience in dialysis.

- Knowledgeable of all dialysis modalities.

- Demonstrated leadership competencies and skills for the position, including excellent communication, customer service, relationship development, results orientation, team building, motivating patients, performance management, decision making, and sales presentational skills.
