JOB SUMMARY:

Under supervision, performs the following functions: Transports patients, visitors to and from their residence for medical appointments at CCF. Performs daily preventative maintenance of assigned vehicle. Performs other related duties as assigned.

MINIMUM QUALIFICATIONS:

Ability to read and write sufficient to fill out maintenance check lists. Ability to follow verbal directions. Must possess a
valid Ohio Commercial Ohio License, maintain eligibility of insurability as determined by CCF's insurance carrier's
requirement to operate Foundation vehicles. Typical physical demands include ability to assist physically challenged
patients and visitors in wheelchairs on and off wheelchair left on vehicle. Requires sitting and driving a vehicle for an
extended period of time. Must be able to pass an annual visual and hearing test.
