Responsibilities include:
Coordinate the management of the referral process to ensure a smooth transition of patients to service with PHC. Develop and maintain collaborative relationships between PHC staff and external referral sources.
Qualifications
Current license in Massachusetts as RN, LPN or Therapist.
Minimum of 2 years of home care experience or equivalent medical experience in post-hospital environments.
Local travel required
Computer literate with working knowledge of Microsoft Office Suite and the ability to learn other software applications.
Broad knowledge of various insurances and government reimbursement to home care
Sales and/or marketing experience
Computer literate with working knowledge of hospital based software applications
Highly developed verbal and written communication and presentation skills
Effective problem solving and decision making skills.
Highly developed interaction with a variety of personality types
